I assume all risk and hazards incidental to the conduct of this program and for the transportation to and from this program. I authorize my child’s coach to obtain medical treatment for my child in the event that the parent’s and emergency contact cannot be reached.  Physical must be on file in the school office that was completed no earlier than 1 June 2011.
